- ItemBilateral locked posterior shoulder dislocation overlooked for 15 months treated with the modified McLaughlin procedure: A case report(Bayçınar Tıbbi Yayıncılık, 2023) Saylık, Murat; Gökkuş, KemalNeglected bilateral posterior shoulder dislocation is a rare injury caused primarily by an epileptic seizure. The injury is usually associated with a reverse Hill-Sachs lesion in the anteromedial aspect of the humeral head. The modified McLaughlin technique may avoid instability and osteoarthritis when 20 to 40% of the articular surface is affected by reverse Hill-Sachs. In this article, we present the clinical results of a case overlooked in the literature for the longest time, i.e., for 15 months. A 46-year-old male patient was receiving treatment for epilepsy for five years. There was no fall or trauma in the four seizures he had during this time. The last seizure was 15 months ago. When the patient presented to our clinic, both shoulders were symmetrically deformed, the anterior shoulder contour disappeared (empty socket sign), and there was a loss of upper contour. The computed tomography (CT) scan revealed a posteriorly locked dislocation with a reverse Hill-Sachs lesion in 32% of the left shoulder and 36% of the right shoulder. We applied the modified McLaughlin procedure to the dominant right shoulder and, two months later, we used it to the left shoulder (with a graft taken from the anterior superior iliac spine). At one-year of follow-up, both shoulders were moderately functional: forward elevation left 70° and right 50°, abduction left 40° and right 60°, and internal rotation: the back of the hand could touch the fifth lumbar vertebra. Meanwhile, the patient did not suffer from recurrent dislocation. The pre- and postoperative Constant-Murley Scores for the right and left shoulder were 30/52 and 11/48, respectively. Although the operational outcomes using the modified McLaughlin technique were not ideal, with no recurrence, the patient seemed to be satisfied with this outcome. In conclusion, in neglected locked shoulder fracture-dislocations, the modified McLaughlin technique is a method that can respond to the pathophysiology by eliminating reverse Hill–Sachs lesion.
- ItemComparison of clinical and radiological results of lateral retinacular release or lateral retinacular lengthening methods combined with medial retinaculum plication in patellofemoral instability(Cureus INC, 2022) Saylık, Murat; Bilgin, Yücel; Atıcı, TeomanIntroduction In this study, we aimed to compare the clinical and radiological results of patients who underwent medial retinaculum plication (MRP) combined with lateral retinacular release (LRR) or lateral retinacular lengthening (LRL) with the diagnosis of patellofemoral (PF) instability. Methods In our study, we retrospectively analyzed 75 knees of 75 adult patients (43 females and 32 males) who underwent MRP+LRR or MRP+LRL due to PF instability without osseous pathologies. Patients were divided into two groups (MRP+LRR and MRP+LRL) according to the surgical method. The clinical and radiological results of the two groups were compared. Results MRP+LRL surgery was performed on 45 knees and MRP+LRR surgery on 30 knees. The mean age was 26.5 (18-43) years. There was no significant difference between the two groups in the change in patellar lateral shift (PLS) (p=0.429) and congruence angle (CA) (p=0.218) values. However, there was a significant difference between the two groups in the change in patellar tilt angle (PTA) (p=0.009) and lateral patellofemoral angle (LPFA) (p<0.001) values. The change in PTA and LPFA values was higher in the MRP+LRL group. There was no significant difference between the two groups in terms of pre-operative and post-operative Lysholm knee scoring scale (p=0.205, p=0.228), Kujala pain scale (p=0.393, p=0.596), and Tegner activity level scale values (p=0.121, p=0.899). Conclusions MRP+LRR or MRP+LRL provided successful results for correcting the instability in PF instability without osseous pathologies such as patella alta, tibial tubercle-trochlear groove (TT-TG) dysplasia, trochlea dysplasia, genu valgus, and tibial-femoral torsion. While PTA and LPFA values improved more with the MRPLRL method, clinical results were similar in both methods.
- ItemEnhancing municipal solid waste management efficiency through clustering: a case study(Taylor and Francis, 2024-11) Çil, Sedat; Karaer, Feza; Salihoğlu, N. Kamil; Tabansız Göç, Gülveren; Çavdur, Fatih; 409903This study leverages real-time datasets generated through IoT technology and smart city applications to enhance solid waste management in Yalova Province, Turkey. By integrating these datasets with the municipality’s Geographic Information System (GIS) using the ITRF/96 3 UTM X Y Coordinate System, a dynamic waste collection framework was established. The K-Means clustering algorithm was employed to determine the optimal waste container placement, considering capacities of 550, 800, 1,000, and 3,000 liters and walking distances of 50–100 ms. Results indicated that 1,000 and 3,000-liter containers with a 100-m walking distance maximized collection efficiency. Replacing 484 traditional containers with 105 units of 3,000 liters reduced total routes by 34%, transport costs by 42.2%, and CO2 emissions by 33.5%. The study underscores the importance of integrating GIS and IoT technologies for real-time waste management, aligning with the UN’s Sustainable Development Goals (SDG 11 and SDG 13). By combining data-driven decision-making with urban sustainability practices, it offers a replicable model for municipalities seeking to reduce costs and environmental impacts in waste collection.
- ItemInterventional therapy of acute coronary syndromes in very old patient population and results of 2 years follow‑up(Springer, 2023) Boyraz, Bedrettin; Peker, Tezcan; Akgümüş, Alkame; Balun, Ahmet; 313898Background Research on cardiovascular treatment options and prognosis in very old age groups of patients is warranted. In our study, we evaluated and followed up on clinical conditions on admission and comorbidities of patients older than 80 years who were admitted to our hospital with acute myocardial infarction and shared our findings. Results A total of 144 patients were included in the study, with a mean age of 84.56 ± 5.01 years. No complications resulting in death or requiring surgery were observed in the patients. All-cause mortality was found to be related to heart failure, chronic pulmonary disease shock, and C-reactive protein levels. Cardiovascular mortality was correlated to heart failure, shock on admission, and C-reactive protein levels. No significant difference in mortality was observed between Non-ST elevated myocardial infarction and ST-elevation myocardial infarction. Conclusions Percutaneous coronary intervention is a safe treatment option with low complication and mortality rates in very old patients with acute coronary syndromes.

Bu çalışmada, Öğrenci-Danışman Atama (ÖDA) problemi için öğrenci ve danışman tercihlerine ek olarak; (i) danışman kapasitelerinin dikkate alınmadığı (ii) danışman kapasiteleri olarak bireysel danışman taleplerinin ve (iii) danışman kapasiteleri olarak ortalama iş yükünün dikkate alındığı durum olmak üzere üç farklı problem kurgusu ele alınmıştır. Her bir problem kurgusunun çözümü için matematiksel programlama modelleri ve bir sezgisel algoritma önerilmiştir. Önerilen yaklaşımın geçerliliğin test edilmesi amacıyla küçük ve büyük ölçekte olmak üzere iki farklı veri kümesi üretilmiştir. Bu veri kümeleri kullanılarak önerilen matematiksel programlar ve sezgisel algoritma, öğrencilerin ve danışmanların tercihlerinin farklı ağırlıklandırma stratejileri için çözülmüştür. Farklı problem kurguları ve atama stratejileri için elde edilen öğrenci-danışman atamaları, öğrencilerin ve danışmanların memnuniyet düzeyleri cinsinden tanımlanan performans ölçütlerine göre analiz edilmiştir. Buna ek olarak, matematiksel programlama modeli ve sezgisel algoritmanın sonuçları kıyaslanmıştır. Bu çalışmanın öne çıkan unsuru, ÖDA problemi için farklı problem kurguları ve atama stratejilerinin ele alınması ve bu durumlar için elde edilen sonuçların, karar vericilerin farklı bakış açılarını yansıtacak şekilde kapsamlı olarak analiz edilmesidir.
- ItemPure-Cubic Optical Soliton Solutions of the Nonlinear Schrödinger Equation Including Parabolic Law Nonlinearity in the Absence of the Group Velocity Dispersion(Springer, 2025-03) Ozisik, Muslum; Altun Durmus, Selvi; Secer, Aydin; Bayram, Mustafa; 226795This article investigates the third-order dimensionless nonlinear Schr & ouml;dinger equation with a parabolic law media term, while deliberately excluding the group velocity dispersion term, which typically governs the propagation of ultrashort pulses. The Generalized Kudryashov approach, a powerful and novel technique, is applied for the first time to obtain pure-cubic optical soliton solutions for this model. Using this method, bright, kink, and dark soliton solutions are derived. To illustrate the dynamics and physical properties of these solutions, 2D, contour, and 3D visualizations are presented. In particular, 2D plots with carefully selected parameter values are provided to investigate how the presence of the parabolic law media term and the absence of the group velocity dispersion term influence soliton behavior. The results clearly demonstrate the physical relevance of the model and emphasize the effectiveness of the Generalized Kudryashov approach as a reliable technique for obtaining analytical solutions to the equation under consideration.
- ItemTesting the Forecasting Power of Statistical Models for Intercity Rail Passenger Flows in Turkey(Sage, 2024-11) Ekici, Üsame; Tüydeş Yaman, Hediye; Şendil, NuriWhile going through a major rail transformation, it is important to develop reliable estimation models for rail passenger flows (RPFs) in Turkey. There are two main approaches in RPF estimation, regressions and autoregressive integrated moving-average (ARIMA) models, both of which were in this study developed using the daily RPF data for the period 2011–2015. The ARIMA models (with some variations) were used to forecast first the daily flows in 2016, during which travel restrictions for summer resulted in reduced volumes, successfully captured in the updated ARIMA model. The regression models predicted the expected demand during the restrictions, enabling evaluation of the impact of restrictions, which also showed the models’ power over the longer term. The forecasts were extended to 2017, 2018, and 2019 data. The regression results produced more reliable forecasts over the long term, whereas more accurate predictions were obtained by ARIMA-Sliding (FA-Sld) for short-term planning purposes.
- ItemThe mediation role of coping with stress in the relationship between psychological flexibility and posttraumatic growth in cancer patients(Sage, 2024-10) Akcan, Gizem; Bakkal, Bekir Hakan; Elmas, Özlem; 192722Objectives: Active and adaptive coping strategies have been identified as factors that can assist cancer patients in effectively managing and overcoming the crisis, leading to the potential for post-traumatic growth during the diagnosis and treatment stages. Psychological flexibility is another factor that can protect individuals and promote more adaptive reactions to traumatic situations. The study aims to examine the investigation of the mediation role of coping with stress in the relationship between psychological flexibility and posttraumatic growth in cancer patients. Methods: This study was conducted with 97 (63 women, 34 male) participants aged between 38 and 82 who are literate and diagnosed with breast cancer or gynecological cancers (endometrial, cervical, vulvar) undergoing radiotherapy planning at the Radiation Oncology Clinic of Zonguldak Bülent Ecevit University Hospital. Demographic Information Form, Psychological Flexibility Scale, Post-Traumatic Growth Inventory, and Coping Styles Scale were applied to the participants. The mediation effects were interpreted using bootstrap confidence intervals and VAF values. Results: This study showed that the active coping styles partially mediated the relationship between psychological flexibility and posttraumatic growth. Conclusion: In this regard, by organizing psychoeducational programs and interventions aimed at increasing psychological flexibility for individuals diagnosed with cancer or undergoing treatment, individuals can be enabled to cope with stress more actively, and thus post-traumatic growth can be achieved.
