- ItemHow close are interprofessional debriefers to meet the INACSL standards of debriefing practices? a qualitative study(2023-12) Özata, Kevser; Caz, Büşra; Sarmasoğlu Kılıkçıer, Şenay; 167438Aim: This study aims to determine whether the debriefings performed by different health professions for nearly a decade meet the INACSL standards of best debriefing practices. Methods: In order for learning to occur in simulation, there must be a adequate debriefing session. An adequate debriefing should be based on the criteria described in the standards of best practices published by the International Nursing Association for Clinical Simulation and Learning (INACSL). A high quality, adequate, and effective debriefing must be carried out in accordance with each standard. Results: Most of the debriefers are female. Over half had two years or less experience with simulation, and a quarter had training in simulation or debriefing. The results are presented in five contexts (Competent Debriefer, Environment, Effective Debriefing, Theoretical Framework, and Objectives and Outcomes.), which conform to the four standards covered by the INACSL standards of best practices. Conclusions: Except for the feedback, the debriefings do not meet the INACSL standards of best practices. However, the debriefers are aware of the importance of debriefing and are willing to improve their skills. Most of the debriefers have learned debriefing from observation or the literature. With this study, it was ensured that debriefers gained awareness in terms of INACSL standards. At the same time, this study will also provide awareness about the standards for the literature.
- ItemMultisource reflections on assessment of nursing students' first clinical practice and predictions for 360-degree assessment: a qualitative study(Hacettepe Üniversitesi, 2023-12) Bayzat, Çiğdem; Sarmasoğlu Kılıkçıer, Şenay; 167438Aim: This study was conducted to explain the current situation regarding the assessment of nursing students during their first clinical practice and to determine the views on 360-degree assessment, an innovative approach, from a multi-source perspective. Material and Methods: In this qualitative case study, individual interviews were conducted with nine instructors, focus group interviews were conducted with twenty-three nursing students, and documents were examined. Data were collected between 7 August 2019–22 November 2020. The data were analyzed in line with the descriptive analysis approach. Results: While reflections on the current situation were generated in line with the findings obtained from three data sources, the context of predictions on 360-degree assessment was generated in line with the findings obtained from interviews with students and instructors. Reflections on the current situation context are analyzed under assessors, competencies, methods and tools, feedback, challenges, and suggestions themes. Predictions on 360- degree assessment context are analyzed under assessors, frequency, tools, setting, benefits, and challenges themes. Conclusion: According to the results of the data triangulation, the most important issues regarding the assessment of nursing students during first clinical practice were the high student/instructor ratio and the fact that only instructors are officially assessors. Instructors and students approached positively to the 360-degree assessment, but they also shared some concerns.
- ItemYoğun bakım hemşirelerinin yaşam sonu bakıma yönelik algıladıkları engellerin ve destekleyici davranışların belirlenmesi(Hacettepe Üniversitesi, 2023-12) Ağgün Yavuz, Güler; Sarmasoğlu Kılıkçıer, Şenay; 167438Amaç: Bu araştırmanın amacı yoğun bakım hemşirelerinin yaşam sonu bakım sunarken algıladıkları en önemli engellerin ve destekleyici davranışların belirlenmesidir. Gereç ve Yöntem: Araştırma kesitsel tanımlayıcı türde olup, evrenini Ankara Şehir Hastanesi’nde yoğun bakım ünitesinde çalışan 278 hemşire oluşturmuştur. Bulgular: Yoğun bakım ünitesi hemşirelerinin yaşam sonu bakım sunmaya yönelik algıladıkları en önemli ilk üç engel: “Hastanın kötü prognozunun aile bireylerince kabullenilmemesi”, “Hastanın ölüm anında aile üyelerinin onunla olmaması” ve “Aile bireylerince yaşam kurtarıcı tedbirler ile etkilerinin anlamının bilinmemesi” iken; algıladıkları en önemli ilk üç destekleyici davranış: “Hastanın ölümünden sonra aile üyeleriyle hekimlerin görüşmesi”, “Aile üyeleri tarafından imzalatılacak olan tüm evrakların sağlık çalışanı tarafınca üniteden ayrılmadan hazırlanması” ve “Hastanın ölümünden etkilenen hemşirenin bir süreliğine ayrılması durumunda diğer hastalar için hemşirelerin sorumluluklarını yerine getirmesi” olarak belirlenmiştir. Sonuç: Yoğun bakım ünitesi hemşireleri tarafından yaşam sonu bakım sunmaya yönelik algılanan en önemli engeller hastaların aileleri ile ilgili olup, algılanan en önemli destekleyici davranışlar ise sağlık çalışanları ile ilgilidir. Ailelere hastalarının kötü prognozunu kabul etmelerini kolaylaştırıcı (psikolojik destek vb.) olanakların sunulması ve hekimlerin yaptıkları bilgilendirmeyi sağlayan koşulların anlaşılmasına ve devamlılığının sağlanmasına yönelik araştırmaların yapılması önerilmektedir.
